Welcome to the Health Information Form page at Venerini Academy. By completing the Health Information Form, you are helping us maintain accurate records and address any medical needs or concerns effectively. ALLERGY information:
We are currently a NUT FREE school. This policy is subject to change pending students enrolled with allergies.
Please refer to our posted information regarding COVID-19 protocol/what to do if your child is sick, and our vaccination policy. If you did not upload a recent physical and immunization schedule with your enrollment packet, please give one to the nurses office before the start of school. Forms may be emailed to nurse@veneriniacademy.com , faxed at 508-754-6050, or dropped off during school hours.
